 BLAYLOCK,
Michael Dale Age 44 of Madison. Passed away on October 11, 2011. He
was employed by the Kroger Co. as a meat cutter for 15 years and
loved to play the banjo. He is preceded in death by his father,
Clayton Blaylock. He is survived by his wife of 24 years, Shelley
Blaylock; mother, Mary Blaylock; daughter, Kelley Blaylock; sons,
Matthew Blaylock and Brandon Blaylock; brother, Ricky (Ruby)
Blaylock; sister, Barbara Charlmers; and grandchildren, Kaylie
Richardson and Maddison Blaylock. Visitation will be held on
Wednesday, October 12, 2011 from 2-8 p.m. and a funeral service will
be held at 2 p.m., Thursday, October 13, 2011 with visitation two
